Comments:
An unannounced monitoring inspection was conducted from 7:15 am - 10:00 am. There were 26 children in care being supervised by 4 staff members. The children were observed participating in table top activities and structured center play. Lunch served today is fish on wheat bean, peas, peaches, and milk. Ten children's records were reviewed and in compliance. Five staff records were reviewed. The most recent emergency escape drill was conducted 4/20/2017 and the most recent shelter in place was conducted 2/24/2017. Six medications were reviewed. There are currently three children in care who are MAT (Medication Administration Training) certified. Standard #: 22VAC40-185-160-C
Description: Based on review of five staff records it was determined that not all staff obtained documentation of a TB (Tuberculosis) screening every two years. Evidence: The most recent TB screening on file for Staff A was dated 8/29/2014.

Plan of Correction: Staff A will obtain a TB screening and submit documentation to administration.

Standard #: 22VAC40-185-510-A
Description: Based on observation of six medications it was determined that not all mediation accompanied parental authorization. Evidence: Child A had an emergency medication at the center that was not accompanied by a medication authorization form signed by the parent of physician.

Plan of Correction: Administration will request a medication authorization form signed by the physician and parent to accompany the emergency medication for child A.

Standard #: 22VAC40-185-510-C
Description: Based on observation of six medications it was determined that the center did not follow all procedures for preventing the use of outdated medication. Evidence: An emergency medication for Child B had an expiration date of October 2016.

Plan of Correction: Administration will request up to date medication from the parents of Child B.
