Discussed with the Director: Organizing the accident report and logging them monthly in order to see if you find a pattern to the accidents. They are logged in order for you to review and to find if there may be ways to prevent some of the accidents from happening.
An Unannounced Renewal Inspection was conducted beginning at 3:50 and ending at 5:15. At 4:00, there were 40 children with 4 staff and the Director. Children were observed at arrival, having snack and then they will do homework and quiet activities. Reviewed staff files, children's files - complete Reviewed medications - complete Emergency escape drills and shelter in place drills are up to date as are health and fire inspections. A Subsidy Health and Safety Inspection was conducted at the end and during the Licensing Inspection. Standard #: 22VAC40-185-550-M Description: Based on a review of 30 accident reports, it was determined that the report forms were not all complete. Evidence: The following information was not completed on some accident reports: date signed by Director; parent signature; date, time and how the parent was notified; information on what the injury was, where it happened and who witnessed. Plan of Correction: The Director will assure that the accident report forms are organized, reviewed and logged. The Director will review the forms to assure that they are complete before filing them.
