Technical assistance was provided in the following areas of the standards: 22VAC40-185-(4)-330 (Play areas); 22VAC40-185-(4)-270 (Building maintenance)

Comments:
An unannounced monitoring inspection was conducted on 5/31/17 from 8:50am - 2:25pm. During the inspection there were 51 children ages three months to five years old in care with 12 staff. A tour of the facility was conducted and children were observed in a variety of activities in the classrooms, during rest period, and eating lunch. Records were reviewed for six children and eight staff. There was no medication at the facility. Emergency procedures and emergency supplies were reviewed. Areas of non-compliance are identified on the violation notice, and were discussed during the exit interview.

Violations:
Standard #: 22VAC40-185-130-B
Description: Based on a review of six children's records, it was determined that the facility did not ensure that they obtain documentation of additional immunizations once every six months for children under the age of two years. Evidence: 1. The record for child #2 (date of birth 11/17/14) contained an immunization record that was dated 11/30/15. 2. Staff #4 (Program Director) confirmed that they did not receive the required additional immunizations for child #2.

Plan of Correction: The facility responded: The parent of child #2 will be asked to provide an updated immunization record.

Standard #: 22VAC40-185-340-D
Description: Based on observation, interviews, and a review of staff records, it was determined that the facility did not ensure that in each grouping of children at least one staff member who meets the qualifications of a program leader or program director shall be regularly present. Evidence: 1. Staff #5 was working alone in the Pre-K classroom with ten children that were four years old. 2. Staff #6 was working alone in the School Age classroom with seven children that were four years old to five years old. 3. The record for staff #5 and staff #6 did not contain documentation to demonstrate that either staff was qualified as a program leader. 3. Staff #4 (Program Director) reviewed the record for staff #5 and staff #6, and confirmed that there was no documentation to demonstrate either staff was program leader qualified

Plan of Correction: The facility responded: Staff #5 and staff #6 will not work alone until they complete all of the training required to be qualified as a Program Leader.

Standard #: 22VAC40-185-500-B
Description: Based on an observation of the diapering process, it was determined that the facility did not ensure that disposable diapers are disposed in storage system that is either foot operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. Evidence: 1. Examination of the trash can use for disposal of diapers in the Toddler classroom revealed that the foot pedal did operate properly to lift the lid. 2. When asked how long the trash can had been broken, the staff present in the classroom stated that the trash can had been broken for a couple of days.

Plan of Correction: The facility responded: A working foot operated trash can was placed in the Toddler Classroom during the inspection.

Standard #: 22VAC40-185-550-D
Description: Based on a review of documentation, it was determined that the facility did not ensure that a monthly practice evacuation drill is conducted every month. Evidence: 1. The emergency drill log did not have documentation that an emergency evacuation drill was completed December 2016 and April 2017. 2. Staff #4 (Program Director) confirmed that an emergency evacuation drill was not completed in December 2016 and April 2017.

Plan of Correction: The facility responded: A practice emergency evacuation drill will be completed each month and recorded on the emergency drill log.
