An unannounced monitoring inspection was completed on 06/01/2017. There were 136 children ages 5 months-6 years in care during the inspection. There were 28 direct care staff and additional support staff on-site, including the Director and Assistant Director present during the inspection. The inspector reviewed 5 of the 140 children's records, 6 of the 45 staff records and 4 of the approximately 12 medications during the inspection. The children were observed during free choice time, story/circle time, outdoor play, lunch and nap. Hand washing, diapering, meal service and medication administration was also observed. Emergency procedures to include evacuation drills and first-aid kits were reviewed. The inspector discussed allergies, qualification to administer medications, diapering vs. food services gloves, labeling all items from home, and areas of non-compliance at the exit meeting. Standard #: 22VAC40-185-270-A Description: Based on observations, the facility failed to ensure that all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition as stated in the standards. Evidence: 1. In the Rainbow Classroom there is a area located directly against the diapering station with peeling paint; there is an area approximately two feet long and three inches wide where the paint is missing and the plaster is exposed and an area approximately three inches in diameter of paint that can be peeled from the wall. 2. The covered porch has a plastic toy box with a cracked and broken lid that has sharp edges and pinch points. 3. There is a tricycle in the outdoor play space with a missing pedal 4. There is a tricycle in the outdoor play space that is missing the rubber handle grips. Plan of Correction: The Director stated that they have a painter coming to paint various areas of the center. The toy box will be discarded and the tricycles will be repaired. The staff will be asked to inform the Director if areas and equipment need attention.
Standard #: 22VAC40-185-280-B Description: Based on observations, the facility failed to ensure that all hazardous substances such as cleaning materials, are kept in a locked place to prevent access by children. Evidence: 1. During observations of the outdoor play space bathroom, the Inspector opened an unlocked cabinet under the sink in the bathroom and found an unlabeled spray bottle that smelled of a pine cleaning solution. Plan of Correction: Cleaning materials will be moved to a locked cabinet. The cleaning staff will be informed to not leave hazardous substances in unlocked cabinets.
Standard #: 22VAC40-185-280-G Description: Based on observations, the facility failed to ensure that when hazardous substances are not kept in original containers that the container clearly indicates the contents. Evidence: 1. During observations of the outdoor play space bathroom, the Inspector opened an unlocked cabinet under the sink in the bathroom and found an unlabeled spray bottle that smelled of a pine cleaning solution. Plan of Correction: The spray bottle will be labeled today. The Director will inform the cleaning staff to label all spray bottles.
Standard #: 22VAC40-185-330-B Description: Based on observations, the facility failed to maintain resilient surfacing for all playground equipment as stated in the standards. Evidence: The metal slide that is located on the hillside does not have the required amount of resilient surfacing for the fall/trajectory zone, the wood mulch that is used as resilient surfacing has grass growing throughout all areas of the fall/trajectory zone. Plan of Correction: Mulch will be added to the area to meet the requirement.
Standard #: 22VAC40-185-500-B Description: Based on observations, the facility failed to ensure that all diapering areas have a nonabsorbent surface for diapering or changing as stated in the standards. Evidence: 1. The diaper changing table in the Orchard Room had an area on the edge of the changing pad that was missing the plastic covering exposing the nonabsorbent pad. 2. The diaper changing table in the Purple Room had a tear in the changing pad that exposed the nonabsorbent pad. Plan of Correction: The changing table pads will be replaced in both rooms. The changing pads will be ordered today.
Standard #: 22VAC40-185-560-G Description: Based on observation, the facility failed to ensure that when food is brought from home it is clearly dated and labeled in a way that identifies the owner. Evidence: 1. Four lunch boxes were observed to not be dated during lunch service in the Pink Room. 2. Two of those four lunch boxes were observed to be unlabeled as to not clearly identify the owner. Plan of Correction: The teachers will check the lunch boxes daily to ensure that all are labeled and dated.
