Violations:
Standard #: VENDSUB-000-006-B-11
Description: Based on record review and staff interviews, the facility failed to obtain a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: The record for Child #3 indicated a diagnosed food allergy, an allergy to peanuts, the facility does not have an allergy care plan for this child.

Plan of Correction: A form for allergy care plan was developed during the inspection that parents/physicians will be required to completed for all children with diagnosed food allergies.
