Technical assistance was given regarding the clear documentation of immunization records and the required time frames and physicians statements/ signatures.
An unannounced Renewal Inspection was conducted on June 1, 2017 from approximately 8:55 a.m. until 11:45 a.m. Upon entrance there were 33 children in the "Kids" program and 4 staff present with children. There were no children on site for the "school age/middle school" program. The center's total enrollment is 51 children for the "Kids" program. The record review sample consisted of 6 children's records and 5 staff records. Children were observed eating snack, working on shape recognition activities, coloring and art projects. Eight medications were reviewed. The results of the inspection were reviewed with the center director. Areas of non-compliance are noted on the violation notice. Please complete the columns for ?description of action to be taken? and ?date to be corrected? for each violation cited on the violation notice, and then return a copy to the licensing office within 5 business days of receipt. Standard #: 22VAC40-185-270-A Description: Based on observation and staff interviews, the licensee did not ensure areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin. Evidence: During the inspection on June , 2017, there is a metal border on the brick wall on the outside play area that has 4 sharp, metal protruding areas and they pose a safety risk to the children in care. Plan of Correction: The center supervisor and facility supervisor will ensure the outdoor play area is maintained and safe.
Standard #: 22VAC40-185-350-E-4 Description: Based on observation, center documentation and staff interviews, the licensee did not ensure that for for children from three years to the age of eligibility to attend public school, five years by September 30 there was one staff member for every 10 children. Evidence: During the inspection on June 1, 2017, the center did not ensure the required ratio of 1 staff for every 10 children was maintained from 8:32 a.m. until 8:53 a.m. 1) At 8:32 a.m. child #4 entered the program and there were 31 children with 3 staff 2) At 8:42 a.m. child #6 entered the program and there were 32 children with 3 staff 3) At 8:53 a.m. a fourth staff members arrived and there were 33 children with 4 staff members. Plan of Correction: The center supervisor will ensure the staff are scheduled and arriving at the appropriate times to ensure proper ratios are met.
