An unannounced Subsidy Health and Safety Inspection was conducted on June 1, 2017 from approximately 8:55 a.m. until 11:45 a.m. Upon entrance there were 33 children and 4 staff present with children. The record review sample consisted of 6 children's records and 5 staff records. Children were observed eating snack, working on shape recognition activities, coloring and art projects. Description: Based on record reviews and staff interviews, the vendor did not ensure all staff providing care to children shall have within 30 days of the date of employment or 90 days from subsidy vendor approval CPR and first aid training: Evidence: During the inspection on June 1, 2017, the records for five staff were reviewed and it was determined there was not documentation that staff #1, #5 and #3 had obtained CPR and first aid training. The hire dates for staff #1 was 6-18-14, staff #2 was 4-10-08 and staff #3 was 10-22-13. The center director verified this during the inspection.

Plan of Correction: Newport News Parks and Recreation Tourism maintains CPR & first aid certifications of all in charge staff. Administration will continue to work towards certification of all staff.

Standard #: VENDSUB-000-031-A
Description: Based on documentation review and staff interviews, the vendor did not ensure there were procedures to address reuniting children with parents or authorized person designated by the parent to pick up the child during a shelter-in- place and lockdown. Evidence: During the inspection on June 1, 2017, there were not reunification procedures documented for shelter-in-place and lockdown.

Plan of Correction: The center supervisor will ensure that reunification plans are included in the emergency plan.
