Comments:
An unannounced monitoring inspection was conducted from 8:45am-10:30am. Upon arrival there were 64 children present with 8 staff present. A sample of 3 staff and 5 children's records were reviewed in addition to 4 medications. Children were observed during circle time, preparing for an art activity and during computer class.

Violations:
Standard #: 22VAC40-185-240-C
Description: Based on a record review the center did not have documentation of the required 16 annual training hours for staff. Evidence: 1. Staff #1 did not have any annual training documented in 2016. 2. Staff #2 had 4 hours of annual training documented in 2016. 3. Staff #3 had 14 hours of annual training documented in 2016.

Plan of Correction: This was an administrative oversight. All staff have received the required training hours on 2016, however the director did not document it in the staff records. The director will ensure all training hours are documented moving forward.

Standard #: 22VAC40-185-270-A
Description: Based on observation the center failed to ensure areas and equipment of the center were maintained in a safe condition. Evidence: In the school age portable there was a large backed television that was not secured to a rolling cart, posing a tipping hazard.

Plan of Correction: The director will remove the television from the classroom. If it is replaced it will be secured to prevent tipping.

Standard #: 22VAC40-185-280-B
Description: Based on observation the center failed to ensure hazardous substances were kept in a locked place using a safe locking method. Evidence: In the 3 year-old room the cabinet above the hand washing sink was not locked. It contained a bottle of bleach water and a disinfectant spray.

Plan of Correction: The screw to the lock was not secured causing the door to open. The director repaired the lock and the cabinet door is not secured and locked.

Standard #: 22VAC40-185-510-A
Description: Based on a record review the center failed to ensure all requirements were met when administering prescription Evidence: Child #1 had a prescription for albuterol that did not have written permission by the parent to administer. The medication was administered on 04/07/17, 05/03/17 and 05/17.

Plan of Correction: This is the grandson of a staff member. The parent's signature will be obtained today at pick-up.
