Comments:
The licensing inspector conducted an unannounced complaint inspection on 06/02/17 in response to a complaint that was received in the licensing office on 06/01/17 regarding allegations in the area of supervision. Interviews were conducted with staff , records were reviewed and observations of the program were made relating to the allegation. The information gathered during the investigation supports the allegations, so the complaint is determined to be VALID.

Description: Based on interviews and a record review the center failed to ensure when children are regularly in ongoing mixed age groups, the staff-to-children ratio applicable to the youngest child in the group shall apply to the entire group. Evidence: On 06/01/17 the school-age room was out of ratio from 6:44am -7:13am until staff #4 entered the room. The youngest child in the group was 2 years-old. The ratio for 2 year-old children is 1 staff for every 8 children. 1. At 6:44am there was 1 staff #3 with 10 children ages ranging from 2-6 years. 2. At 6:45am there was 1 staff #3 with 12 children ages ranging from 2-6 years. 3. At 6:48am there was 1 staff #3 with 13 children ages ranging from 2-6 years. 4. At 6:53am there was 1 staff #3 with 14 children ages ranging from 2-6 years. 5.At 6:55am until 7:13am there was 1 staff #3 with 10 children ages ranging from 2-6 years.

Plan of Correction: Staff schedules have been changed to ensure that we are in compliance with ratios in the multi age room each morning, effective June 5, 2017. Management will continue to monitor Face to Name sheets daily to ensure that staffing patterns are developed and changed as needed to ensure compliance.

Description: Based on interviews and a record review the center failed to ensure the ratio of one staff member for every four children for children ages birth to the age of sixteen months. Evidence: On 06/01/17 the infant room was out of ratio from 6:43am-7:20am when staff #2 entered the room. 1. At 6:43am there was 1 staff #1 with 5 infants. 2. At 6:51am there was 1 staff #1 member with 6 infants 3. At 6:56am there was 1 staff#1 member with 8 infants
