Comments:
An unannounced monitoring inspection was conducted on Thursday, June 8, 2017, from approximately 10:00am until 3:00pm.
The following classroom ratios were observed:
Infant I = seven infants with two staff
Infant II = six infants with two staff
Toddler I = six children with two staff
Toddler II = eight children with two staff
EP1 = six children with one staff
EP11- six children with one staff
Preschool = ten children with one staff
Pre-K = sixteen children with two staff
Observations made today by the licensing inspector included children listening to stories, discussing what lives in the ocean, using various manipulatives, using the sensory table, working on math sheets, counting shells, playing outdoors, lunch and nap time preparation. Staff were providing frequent interactions with children of all ages. Infant staff were observed sitting with the children on the floor while playing and talking.
Lunch: barbecue sandwich, corn and peas, apricots, and milk.
Five children's records, one medication and four staff records were inspected.
All required emergency items were present. A practice evacuation drill was conducted 5/31/17.

Violations:
Standard #: 22VAC40-185-160-A
Description: Based on inspection of staff records, the center did not ensure each individual submitted a negative tuberculosis (TB) screening within 21 days after employment. Evidence: Out of four staff records inspected, Staff # 3, began working at the center 4/13/17 and does not have documentation of a TB.

Plan of Correction: As discussed on June 8, 2017, the Center hired this staff member from another Bright Horizons? location which closed (where the staff member had all of her background checks and TB completed and on file). Prior to staff member?s first day, the Center Director requested that her file be sent over. While the staff member?s file was sent, the background checks and TB were placed in a storage facility when the Center changed ownership. On April 13, 2017, the Center Director placed an expedited order for that specific documentation to be retrieved and will update the Department of Social Services upon receipt of the same. Going forward, the Center Director and her administrative team will treat any transfer employee as a new employee and ensure all related documentation is on file to avoid any lapse.

Description: Based on inspection of staff records, the center did not ensure each person had an original criminal history record report within 30 days of employment.
Evidence:
Out of four staff records inspected, Staff #3 began working at the center 4/13/17 and does not have documentation of an original criminal history record report within 30 days of employment.
