22VAC40-185 SPECIAL CARE PROVISIONS AND EMERGENCIES.
An unannounced inspection was conducted on 5/31/17 from 10:30am till 2:30pm for a complaint that was received in the licensing on 5/5/17 regarding the care and supervision of the children, and injury reporting. Interviews were conducted and documentation was reviewed. Evidence gathered during the investigation supports the allegation, so the complaint is deemed "valid". Violations related to this complaint are listed on the violation notice attached to this summary.
Standard #: 22VAC40-185-550-M Complaint related: No Description: Based on information gathered during the investigation, it was determined that the facility did not ensure that they maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. Evidence: 1. According to the parent, child #1 ( two years old) came home on 4/14/17 with a cut where his ear attaches to his head, approximately 2-3mm deep and 1/4 inch wide. There was no incident report given to the parent and no notification on the "My Day" sheet for child #1. The facility was notified by the parent on 4/17/17 about the injury. 2. According to the parent, child #1 ( two years old) came home on 2/24/17 with a large, green knot on his forehead. There was no verbal or written report given to the parent. 3. When the licensing Inspector reviewed injury reports, there was no injury report for for child #1 available for viewing at the facility for the injury that occurred on 4/14/17, or for the injury that occurred on 2/24/17. 4. When staff #1 (Program Director) was asked if injury reports were completed for these two injuries for child #1, staff #1 stated that she could not find an injury report for either injury . 5. There were five additional injury reports for children that did not include the date, time, or method of parent notification. 6. Staff #1 (Program Director) reviewed the injury reports that the Licensing Inspector reviewed, and confirmed that all of the injury reports did not include all of the required information. Plan of Correction: The faiclity responded: Staff will work to ensure that children will be evaluated at drop off for injuries that might've occurred at home and will investigate with parents to determine cause, documenting aforementioned injuries. Additionally, any injuries that are observed while a student is in care will also be documented with an incident report, and all staff will work to ensure that proper reports are completed, shared with parents, and properly noted of the parent disclosed to, as well as time and date of disclosure.
