Comments:
An unannounced monitoring inspection was conducted on 6/14/17 from 8:40am - 12:30pm. During the inspection there were 104 children ages four months to twelve years old in care with 22 staff. A tour of the facility was conducted and children were observed in a variety of activities in the classrooms, playing outside, during rest period, and eating lunch. Records were reviewed for six children and five staff. Medication, emergency procedures, and emergency supplies were reviewed. Areas of non-compliance are identified on the violation notice, and were discussed during the exit interview.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based a review of seven staff records, it was determined that the facility did not ensure that they maintain and keep at the center a complete record for each staff that contains all required information. Evidence: 1. The record for staff #7 only contained documentation that one reference as to character and reputation as well as competency was checked before employment. 2. The record for staff #6 did not contain the date of employment or the job title. 3. Staff #8 (Program Director) confirmed that the records for staff #6 and staff 37 were not complete.

Plan of Correction: The facility responded: A second reference will be completed for staff #7, and the the record for staff #6 will be updated to include all of the required information. New staff will not be allowed to begin employment until their records are complete.

Standard #: 22VAC40-185-260-A
Description: Based on a review of records, it was determined that the facility did not ensure that an annual fire inspection report from the appropriate fire official having jurisdiction was completed. Evidence: 1. The last completed fire inspection had 5/20/16 listed as the date of inspection. 2. Staff #8 (Program Director) confirmed that the annual fire inspection had not been completed.

Plan of Correction: The facility responded: The fire marshal will be contacted to ensure the annual fire inspection is complete.

Standard #: 22VAC40-191-60-C-1
Description: Based on a review of seven staff records, it was determined that the facility did not deny continued employment of a staff who did not have an original criminal history record report within 30 days of employment. Evidence: 1. The record for staff #1 (date of hire 4/28/17) did not contain documentation of a an original criminal history record report. 2. Staff #8 (Program Director) confirmed that the criminal history record report has not been received for staff #1.

Plan of Correction: The facility responded: A criminal record check was requested for staff #1. We will call the Virginia State police to check the status of the request.

Standard #: 22VAC40-191-60-C-2
Description: Based on a review of seven staff records, it was determined that the facility did not deny continued employment of a staff who did not have a search of the central registry finding within 30 days of employment. Evidence: 1. The record for staff #1 (date of hire 4/28/17) did not contain documentation of a completed search of the central registry finding. 2. The record for staff #6 (date of hire 5/8/17) did not contain documentation of a completed search of the central registry finding. 3. The record for staff #7 (date of hire 4/28/17) did not contain documentation of a completed search of the central registry finding. 4. Staff #8 (Program Director) confirmed that the search of the central registry finding has not been received for staff #1, staff #6, and staff #7.

Plan of Correction: The facility responded: A new search of the central registry for each staff was mailed. We will call the Virginia Department of Social Services to check the status of the request.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation, it was determined that the provider did not ensure that the most recent inspection was posted. Evidence: 1. The most recent inspection report was not posted in the family day home. 2. Caregiver #1 confirmed that the most recent inspection report was not posted and showed where she had filed it by mistake..

Plan of Correction: The facility responded: Corrected during the inspection. The most recent inspection will be posted in the center at all times.
