- Technical Assistance:
Technical assistance today included playground safety rules for children and emergency supplies. Beginning 2014, facilities who care for a child with rectal medication were required to have at least one staff complete the MAT Administration of Emergency Rectal Medications in Child Care Programs (MAT@medhomeplus.org)
An unannounced monitoring inspection took place today from 9:00 - 11:20 am. There were 96 children present today. All classrooms were within ratio. I observed children practicing for their upcoming graduation with the "ABC See You Later" song; involved in dramatic play; playing outdoors before the rain and staff singing in the baby room. Record keeping was well-organized. If you have any questions, please contact email@example.com

Evidence: Staff # 1, who began 4/17/17, did not have available documentation of a TB screening as required. Plan of Correction: Staff member recieved TB test before coming back to work Monday.
Standard #: 22VAC40-185-240-D-1 Description: Baed on a review of records, medications and staff confirmation, no staff is current in the training of the administration of a medication that is on site for Child # 1. Evidence: Child #1 was prescribed a rectal medication beginning 9/15.. Although staff and documentation report an attempt was made for weaning the child off the medication, it has been prescribed and at the center since 3/17 and no staff has completed the appropriate training for this medication. Plan of Correction: CD had training for MAT but will get other MAT trainers to receive training.
Standard #: 22VAC40-185-270-A Description: During today's inspection, some areas and equipment of the center were observed not to be maintained in a clean, safe/operable condition. Evidence: Indoors: On the side of a crib in the infant room was a vent that was very dirty and chipping green paint. Outdoors: A large cable was observed to be stretched across the lower part of an aluminum fence in the playground. It was straight across but with enough space to pose an entrapment hazard at child height. There were also some areas on the Jungle House where the plastic was broken. Plan of Correction: Place work order on 5/22/17 to have facilities team come out to repair fence area. Follow up with repairs needed of the Tree house. Spoke to Facilities about replacing the vent.
Standard #: 22VAC40-191-40-D-1-B Description: Based on a review of a sample of records today, one staff did not have a signed Sworn Statement prior to first day of employment as required. Evidence: Staff # 2, who began 5/22/17, did not have a signed Sworn Disclosure Statement available today. Plan of Correction: Corrected during inspection.
