Technical Assistance:
Discussed the following standards: physical plant, equipment, hazardous substances, and food storage.

Comments:
An unannounced monitoring inspection was conducted today. Observed 30 children directly supervised by 5 staff members. A sample of 2 children's records were reviewed, 2 staff records, and 7 additional staff background checks were reviewed. The children were engaged in various activities, to include: free play, dancing, singing, and infant care. Areas of non-compliance are identified in this report. The exit interview was conducted with the Director. Inspection conducted from approximately 10:00am to 11:45am. Description: Based upon documentation reviewed, a child did not have a physical examination before the child's attendance or within one month after attendance. Evidence: One of the two children's records reviewed did not contain a physical examination (Child A; DOE 09/11/2014).

Description: Based upon documentation reviewed, a child's immunizations record did not meet requirements. Evidence: One of the two children's records reviewed contained immunizations records that were not signed by a physician, his designee, or an official of a local health department (Child A).

Description: Based upon observations made, areas and equipment of the center were not maintained in a safe condition. Evidence: 1) 2 small AA batteries were in the top drawer and a first aid kit was accessible in bottom drawer in the two years old classroom. 2) Gap under wall about 1-2 inches high under the cork board in the three years old classroom. 3) 2 adult scissors were accessible to children, large holes in play ?sink? and ?oven? equipment, causing a potential head entrapment hazard in the four years old classroom. 4) Peeling paint on the window sill, broken wall lining along the left side of the wall with sharp edges in the nap classroom. 5) Red riding car was missing the handle, exposing sharp plastic edges, on the wooden wall by the swings there is a screw that is exposed by about 1-2 inches in the playground.

Description: Based upon observations made, hazardous substances were not kept in a locked place using a safe locking method that prevents access by children. Evidence: 1) 2 Orkin bug traps were on to window sill and accessible to children in the three years old classroom. 2) Lysol wipes were inside unlocked cabinet in the four years old classroom.

Description: Based upon documentation reviewed, background checks were not repeated within the allowable three years time frame. Evidence: 1) Staff B did not have a current Sworn statement or affirmation (SDS) form on file; last SDS form was dated 08/05/2013. 2) Staff C did not have a current SDS form on file; last SDS form was dated 05/27/2013. 3) Staff D did not have the current background checks: child protective service (CPS) check, SDS form, and criminal history (CRC) check on file. Last CPS check was dated 01/07/2014, last SDS form was dated 12/17/2013, and last CRC check was dated 01/15/2014.
