Comments:
An unannounced Subsidy Health and Safety inspection (SHSI) was conducted today. Observed 30 children directly supervised by 5 staff members. A sample of 5 children's records were reviewed and 9 staff records were reviewed. The children were engaged in various activities, to include: free play, dancing, singing, and infant care. Areas of non-compliance are identified in this report. The exit interview was conducted with the Director. Inspection conducted from approximately 10:00am to 11:45am. Standard #: VENDSUB-000-035-D-5
Description: Based upon interview, allergy plans were not carried during transport of children. Evidence: Allergy plans for children with diagnosed food allergies were not carried during transportation.

Description: Based upon documentation reviewed, children's records were incomplete. Evidence: Four of the five children's records reviewed did not contain a written allergy care plan for diagnosed food allergies (Child A, B, C and D).

Description: Based upon documentation reviewed, staff did not complete the Virginia Preservice Training for Child Care Staff within the first 90 days of employment or subsidy vendor approval. Evidence: One of the nine staff records reviewed did not contain documentation verifying that the required training was completed (Staff A).
