Comments:
67 children were present during the inspection. The age range was three months to nine years old. children were observed during outside time, free play. The older children were getting ready for a field trip Ten children's records were reviewed. Inspection time10:00am-1:00pm

Description: Based on review of six staff records, the center did not maintain complete records. Evidence: Records for Staff B, hire date of 5/30/17, and Staff C, hire date of 5/16/17, did not have documentation of two references being checked.

Plan of Correction: Employee is no longer with us other files have been updated.

Standard #: 22VAC40-185-210-A
Description: Based on review of six staff records, the center did not have proof of program leader qualifications for a staff member. Evidence: The records for Staff A, hire date 5/29/17, and Staff B, hire date 5/30/17, did not have documentation of lead teacher qualifications. Staff A and B are currently assigned as classroom lead teachers

Plan of Correction: Teachers will be lead qualified within 30 days of date listed.

Standard #: 22VAC40-185-230-B
Description: Based on observation and staff interviews, the independent contractor was not in sight and sound supervision of a staff member when in the presence of a group of children. Evidence: On June 15, 2017, the licensing inspector observed the soccer coach working unsupervised with a group of children on the playground. There were no background checks on file for this soccer coach. The staff interviewed did not know the name of the soccer coach.

Plan of Correction: Until background check is in and cleared have staff member present, They will not be back until the fall.
