Technical Assistance:

The licensing inspector conducted an unannounced monitoring inspection Thursday, June 15, 2017 from 11:06am to 3:45pm. The children were observed listening to a story, eating lunch and resting on cots. Lunch consisted of turkey sandwich, mixed vegetables, peaches and milk. The lunch menu is posted. All classrooms and playgrounds were inspected today as well as the facility?s mini buses. Four children?s records and three employee records were reviewed during this inspection. The center administers medication and it was reviewed. Five injury reports were reviewed and an injury prevention plan is completed monthly. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 05/22/2017 Last shelter-in-place drill: 03/31/2017 & 05/05/2017 Last fire inspection: 10/18/2016 Last health inspection: 11/18/2016 Total enrollment today: 78 Today, the following child to staff ratios were observed: Infants ?6:2 Toddlers ?10:2 Twos ? 17:3 Threes ?10:1 Pre-K-10:1 School Age ?25:3 The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-510-N
Description: Based on a review of medication on 06/15/2017, the center did not ensure that when an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days are disposed of by the center. EVIDENCE: 1. The authorization for prescription cream for Child #3 expired on 04/23/2017. The medication was in the medication cabinet at the center. 2. The authorization for prescription cream for Child #3 expired on 04/26/2017. The medication was in the medication cabinet at the center.

Plan of Correction: Will send creams home today, 06/15/2017.

Standard #: 22VAC40-185-540-C
Description: Based on observation on 06/15/2017, the center did not ensure that the first aid kit on the bus contained the required items. EVIDENCE: The first aid kit on the bus did not have tweezers or a cold compress.

Plan of Correction: Will place items on bus today.

Standard #: 22VAC40-185-550-M
Description: Based on a review of injury records on 06/15/2017, the center did not ensure that each record contained the required information. EVIDENCE: Out of a sample of five injury reports: 1. three did not have documentation of the time the parent was notified. 2. two did not have documentation of how the parent was notified. 3. one did not have documentation of the date the parent was notified. 4. two did not have documentation of the treatment provided.

Plan of Correction: Will go over at next staff meeting. Will train staff on how to complete the injury report and the importance of completing the report.

Standard #: 22VAC40-191-40-B
Description: Based on a review of staff records on 06/15/2017, the center did not ensure that no person with a disqualifying background who has not been granted a waiver may work at the facility. EVIDENCE: Staff #3, who has been employed at the center since 03/24/2016, has a felony conviction that is a barrier crime. This conviction was in 1981.

Plan of Correction: Staff #3 will be notified immediately that they cannot service children here at Genesis as of 06/15/2017 due to a nonviolent barrier crime committed in 1981. In addition, human resources will review in detail the Virginia standards the relate to barrier crimes to ensure that an understanding of the standards is clear.
