Comments:
An unannounced monitoring inspection was conducted to investigate a self-reported incident regarding lack of care, protection, and guidance. The inspection was conducted with the director beginning at 10:30 AM and ending at 1:40 PM. Four staff interviews and classroom observations were conducted. One parent interview and child observation was conducted with a member of the local agency. One child?s record and two staff records were reviewed and in compliance. It was determined that there was insufficient corroborating information to substantiate the allegations. Description: Based on a review of documentation and staff interviews, the center did not maintain a written record of children's minor injuries in which the entries are made the day of the occurrence. Evidence: Child A and B were involved in an accident but there was no documentation present in the children's records.

Plan of Correction: The center will document accidents and incidents on the day of the occurrence.
