COMPLAINT # 47457 An investigation took place at the center today following receipt of a complaint. Staff were interviewed, interactions were observed, and records were reviewed. The complaint involved the following standards: 240D4: Child shall always be in the care of a staff member who meets the requirements for administering an emergency medication (such as glucagon.) 340: Supervision: Ensuring the care, protection and guidance of children 430A : Appropriate sized furniture 490C: Communication regarding communicable diseases. 550L: Written injury reports. Due to the lack of a preponderance of evidence, the following standards were found to be in compliance: 340, 490C and 550L. However, the following standards were found to be in violation today. A Violation Notice was given to the center director and a Plan of Correction will be written for 240D and 430A. If you have any questions, please contact me at

Description: Based on record review and staff confirmation, it was found today that there has been a child in care receiving diabetes medication and treatment but that child has not always been in the care of a staff member or independent contractor who meets the state requirements for specialized diabetes medication administration and treatment. Evidence: Child # 1, diagnosed with juvenile diabetes since before beginning care on 2/21/17, was with staff who did not obtain the state -mandated training for diabetes as required. The requirements include that the child always be in the care of a staff member who has completed the state training. One staff, Staff # 2 took the training May 3, 2017. The center is unable to ensure that Child # 1 is always in the care of that staff member.

Description: Based on a review of records and staff confirmation, a furnishing used by the child was not of an appropriate size. Evidence: On 3/13/17, Child # 1 (age 2) was seated in the office. The incident report reads that she slipped out of the chair (an adult-sized chair) while in the office getting tested for glucose readings, and received a bump on her head.
