22VAC40-185 STAFFING AND SUPERVISION.
Standard #: 22VAC40-185-340-A Description: Based on staff interviews and the center's video recording, staff failed to ensure care and protection.
Evidence: On 6/23/2017 Child #1 was left out on the playground alone for approximately 2 minutes. According to the center's policy, staff are required to complete a name to face procedure to ensure that all children are accounted for. This procedure is required at each threshold. Staff #1 and #2 did not complete a name to face check to ensure that all children were accounted for while returning from the playground to the classroom.
Plan of Correction: Staff who were involved in this incident were terminated. All staff were re-trained in the procedures for playground safety and the center's policy on name to face.
Standard #: 22VAC40-185-340-F Description: Based on staff interviews and the center's video recording, staff failed to provide actual sight and sound supervision.
Evidence: On 6/23/2017 Child #1 was left out on the playground for approximately 2 minutes. During the approximate 2 minutes Staff #1 and Staff #2 failed to provide sight and sound supervision for Child #1.
Plan of Correction: Both staff were terminated. All staff were re-trained in playground procedures and staff supervision and the center's own policy on name to face identification.
