An unannounced inspection was conducted on July 5, 2017 to review subsidy and health inspection requirements. Five staff were present with thirty-three children ranging in age from infants through school age children. Six staff and five children's records were reviewed.
Standard #: VENDSUB-000-012-E-1 Description: Based on review, facility did not ensure 3 of 6 staff complete CPR training within 90 days of employment. Evidence: On July 5, 2017, the records for staff #2 (start date 3/13/2017), staff #3 (start date 1/10/2017) and staff #5 (start date 7/14/2014) do not have current CPR certification. Plan of Correction: A mandatory training is scheduled for all staff who do not have current CPR and First Aid training.

Standard #: VENDSUB-000-031-A Description: Based on review, facility did not ensure the emergency preparedness plan has required information. Evidence: On July 5, 2017, the emergency preparedness plan did not address relocation procedures and reunification with parents. Plan of Correction: Program Director will make all modifications and corrections today and review with all staff.
