Comments:
An unannounced monitoring inspection was conducted on July 5, 2017 from approximately 3:00 PM to 6:15 PM. Nine staff were present with one hundred two school age children. Children were observed participating in various small and large group activities as well as eating a afternoon snack. Five staff records and ten children's records were reviewed. Licensing Inspector reviewed supervision, activities, equipment, medication procedures, nutrition, emergency supplies, evacuation and shelter-in-place drill documentation, emergency preparedness plan, fire and health inspection reports and required postings. Violations were reviewed with and verified by the program director.

Standard #: 22VAC40-185-160-A
Description: Based on review, facility did not ensure 1 of 5 staff have a TB test/screening within 21 days of beginning employment. Evidence: On July 5, 2017, the record for staff #5 (start date May 25, 2017) did not have a TB test or screening.

Plan of Correction: It will be re-printed and placed in file.

Standard #: 22VAC40-185-280-B
Description: Based on observation, facility did not ensure hazardous substances are kept locked. Evidence: On July 5, 2017, Foaming Wall Cleaner spray labeled "warnings...keep out of reach of children" was stored on a portable step ladder in the large gymnasium. The spray was not locked and within reach of children.

Plan of Correction: It will be removed today to the locked janitor closet.

Standard #: 22VAC40-185-280-G
Description: Based on observation, facility did not ensure if hazardous substances are not kept in the original containers, the substitute container is labeled with the contents. Evidence: On July 5, 2017, yellow liquid was observed in generic spray bottle that was labeled "water". This was stored on a high shelf in the Skippers room.

Plan of Correction: It will be removed today and re-labeled.

Standard #: 22VAC40-185-560-F
Description: Based on observation, facility did not ensure when meals or snacks are served that they meet USDA requirements. Evidence: On July 5, 2017, the older school age children were served 4 ounces of juice and cheez it crackers for afternoon snack. When juice is provided, 6 ounces of juice shall be served to children ages 6 - 12 years old.

Plan of Correction: Contact will be made with food service provider to provide 6 ounce juice cartons or increase supply if 2 smaller portions are needed.
