An unannounced inspection was conducted on July 5, 2017 to review subsidy health and safety inspection requirements. Nine staff were present with one hundred and two school age children. Five staff records and ten children's records were reviewed.
Standard #: VENDSUB-000-012-B Description: Based on review, facility did not ensure 2 of 5 staff complete the Virginia Preservice Training for Child Care Staff within 90 days of employment of subsidy vendor approval. Evidence: On July 5, 2017, there was not documentation for staff #1 (start date November 2014) and staff #2 (start May 27, 2016) completing the Virginia Preservice Training. Plan of Correction: Certificates will be re-printed and placed in the files.
Standard #: VENDSUB-000-031-A Description: Based on review, facility did not ensure the emergency preparedness plan has all required components. Evidence: On July 5, 2017, the emergency preparedness plan did not have documentation of lockdown procedures and reunification with parent procedures. Plan of Correction: That information will be added to the plan.
