Technical Assistance:
Technical assistance was provided in the following areas of the standards: 22VAC40-185-(3)-210-A (Program Leader Qualifications); 22VAC40-191 (Background checks)

Comments:
An unannounced monitoring inspection was conducted on 7/2/15 from 1:00pm - 2:40pm. During the inspection there were 38 children ages 5 -12 years old in care with five staff. Upon arrival to the facility, the children were observed returning from a field trip. Once in the facility, children were observed eating lunch, playing games, reading books and various other activities in the classrooms. Records were reviewed for five children and five staff. Medication, emergency procedures and emergency supplies were reviewed. Areas of non-compliance are identified on the violation notice, and were discussed during the exit interview.

Standard #: 22VAC40-185-550-M
Description: Based on a review of thirteen injury/accident reports, it was determined that the facility did not ensure that the written record of children's serious and minor injuries included all required items. Evidence: 1. Two of the thirteen injury/accident reports reviewed did not include any future action to prevent recurrence of the injury. 2. Staff #4 (Program Director) reviewed the injury reports and confirmed that two of the injury reports did not include all of the required information.

Plan of Correction: The facility responded: The teachers will be re-trained how to complete an injury report so that all of the required information is included. Center management will review each injury report before signing.
