22VAC40-185 STAFFING AND SUPERVISION.
Licensing Inspector arrived at 10:30 AM and departed at 12:45 PM to investigate a self-reported significant in conjunction with a unannounced monitoring visit. Staff were interviewed and center documents reviewed related to a child being left alone in a restroom. Violations were noted in the area of staffing & supervision.
Standard #: 22VAC40-185-340-A Description: Based on interviews and review of center documents, it was determined that the facility did not ensure that when staff are supervising children, they always ensure their care, protection and guidance. Evidence: 1. On 5/17/17, child #1 was signed into the classroom at 7:56 AM. Staff #1 came on duty in the two-year-old room at 9:27 AM. Eight children were signed into the classroom. At 9:30 AM, she recorded the classroom attendance as seven. 2. At 9:35 AM, the staff #1 took the seven children to the playground. Staff #1 stated that she had checked the bathroom prior to going outside. 3. Staff #1 failed to adequately check the bathroom as demonstrated by the fact that at approximately 9:50 AM, child #1 was found, crying, in a locked bathroom by staff #3. Plan of Correction: Staff were retrained on child supervision and attendance taking procedures on 5/18/17. District Leader will conduct monthly visits to ensure staff are ensuring children's safety and supervision.
Standard #: 22VAC40-185-340-F Description: Based on interviews and review of center documents, it was determined that the facility failed to ensure that children under 10 years of age are always within actual sight and sound supervision of staff. Evidence: 1. On 5/17/17, staff #1 went on duty in the two-year-old classroom at 9:27 AM. At 9:30, she indicated on the Child Supervision Record that child #1 was not present in the classroom. 2. At 9:35 AM, she took the class outside to the playground indicating again, on the same record, that child #1 was not with the group. 3. At approximately 9:50 AM, staff #2 told staff #3 (program director) that she thought she heard come crying coming from the area of the two-year-old room. 4. Staff #3 stated that she went to the two-year-old room and also heard crying. She went to the bathroom and found the door locked with child #1 inside. 5. Staff #1 stated that she had checked the restroom prior to taking the class to the playground Plan of Correction: Management will be conducting a minimum of two checks per day to ensure staff are accurately completing the child supervision record. Staff were retrained on transitioning, head counting and name to face procedures.
