An unannounced monitoring inspection was conducted on July 10, 2017 from approximately 8:00 AM to 9:45 AM. Two staff were present with nine school age children. Children were observed playing freely and watching television. Licensing Inspector reviewed supervision, activities, equipment, evacuation drill documentation, record keeping, emergency supplies and required postings.
Standard #: 22VAC40-185-60-A Description: Based on review, facility did not ensure 2 of 5 children's records has documentation of required information. Evidence: On July 10, 2017, the following was found: 1. The record for child #4 (start date October 3, 2016) does not have documentation of mother's employment phone number and the address for the second emergency contact. 2. The record for child #5 does not have documentation of start date, previous programs attended, programs currently attending, chronic physical problems, allergies and verifying child's identity/age. Plan of Correction: Both of those will be completed today at pick up.
Standard #: 22VAC40-185-70-A Description: Based on review, facility did not ensure 1 of 2 staff records does not have documentation of required background checks. Evidence: On July 10, 2017, the record for staff #2 did not have documentation of completed central registry check. Plan of Correction: A copy was brought during inspection and filed in the staff record.
