22VAC40-185 STAFFING AND SUPERVISION.

Comments:
Licensing Inspector arrived at 2:00 and departed at 3:30 PM to investigate a self reported incident regarding a child being left alone in a classroom. Interviews were conducted. A violation was cited in the area of staffing and supervision.

Violations:
Standard #: 22VAC40-185-340-F
Description: Based on interview and review of center documents, it was determined that the facility did not ensure that all children under the are of 10 years are always within actual sight and sound supervision of staff. Evidence: 1. On 6/2/2017 at approximately 4:05 PM, a four-year-old child was found crying by the school-age teacher in the pre-kindergarten room. 2. The staff and children in this class had gone outside and the child had been in the bathroom. Staff admitted that they had not done the name to face check prior to going outside.

Plan of Correction: All teachers have been retrained on transition practices and how to properly perform name to face checks.
