An unannounced subsidy health and safety inspection was completed with the regular licensing inspection on 7/12/17.
Standard #: VENDSUB-000-012-B Description: Based on record review, the center failed to ensure that the required Virginia Pre-service training was completed by all staff. Evidence: 1. Staff #2 had not completed the VA Pre-service training. Staff #2 hire date was 3/13/15. Plan of Correction: The director will have all staff complete the training and put the certificate of completion in all staff files.
Standard #: VENDSUB-000-012-E Description: Based on record review and interview with the director, the center failed to ensure that all staff had current first aid and CPR training. Evidence: 1. Staff #2 observed working during the inspection did not have current first aid and CPR certification. Staff #2 hire date was 3/13/15. Plan of Correction: The director will schedule first aid and CPR classes for all staff to attend.
Standard #: VENDSUB-000-031-B-3 Description: Based on interview with the director, the center failed to ensure that there were written lock down procedures. Evidence: 1. The director stated that there were no specific written procedures for the center for lock down. There was also no reunification plans for emergency plans. Plan of Correction: The director will develop written lock down procedures based on the required components wit reunification plans for all emergency plans (evacuation, shelter-in-place, lock down).
