Comments:
An unannounced subsidy health and safety inspection was conducted on July 10, 2017, between the hours of approximately 10:30 a.m. and 5:10 p.m. Eighty-three (83) children were enrolled and fifty-nine (59) children were present, with nine staff members, on this date. One additional staff member was on site and assisted, as needed. Subsidy requirements were reviewed for compliance. The exceptions to compliance were noted on the accompanying violation notice. Violations:
Standard #: VENDSUB-000-005-B
Description: Based on review of each classroom's daily attendance record and discussion with staff on July 10, 2017, the program failed to maintain a written record of daily attendance for each group of children that documented the arrival and departure of each child in care as it occurred. Evidence: 1. Review of attendance records, on this date, determined that the attendance records for the infant and toddler classrooms were maintained for each classroom but not for each group of children within each classroom. 2. Staff #1 confirmed the current method of attendance documentation (by classroom and not by grouping of children).

Plan of Correction: Attendance records - we made a form for each area. Has been corrected.

Standard #: VENDSUB-000-012-D
Description: Based on review of six staff records and discussion with staff #1 and #2 on July 10, 2017, the program failed to provide orientation training covering all required topics within seven days of a staff member's employment or the date of subsidy vendor approval. Evidence: 1. Review of six staff records, on this date, revealed that orientation training had not been completed on the topics of prevention of shaken baby syndrome and abusive head trauma (AHT), prevention of sudden Infant death syndrome (SIDS) and safe sleep practices, and the emergency preparedness plan as required by VENDSUB-000-(9)-031. 2. Staff #1 and #2 verified that orientation training for staff did not cover the above topics.

Plan of Correction: Orientation - Will be done on 8/14/17 - in-service day. We will go over new rules.

Standard #: VENDSUB-000-012-E-1
Description: Based on review of six staff records and discussion with staff on July 10, 2017, the program failed to ensure that one staff member had current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of the children in care. Evidence: 1. Review of staff #9's record, on this date, established that the staff member was currently certified only in adult CPR (effective date 5/26/2017). 2. Staff #2 and staff #3 confirmed the current CPR certification.

Plan of Correction: CPR will be done within the next two weeks.
