Technical Assistance:
Licensing inspector will research if plastic see-saw equipment will need mulch.

Comments:
An announced initial inspection was conducted on July 11, 2017 from approximately 9:30am-12:30 pm . Seven classrooms, five toilets and twelve sinks and an outside play area were inspected to be used for the children in care. Facility will provide care for children ages infants through twelve years. The following items were reviewed as a part of the inspection: health and safety, physical plant, required documentation, and emergency supplies, procedures and required postings. The center will not be administering medication at this time. The inspection will be reviewed with the Licensing Administrator regarding issuance of a conditional license. Exit interview conducted with owner and program director.

Violations:
Standard #: 22VAC40-185-40-I
Description: Based on observation and inspection of the facility, the applicant did not develop written procedures for injury prevention. Evidence: During the inspection conducted on July 11, 2017, the center did not have documentation of written procedures for injury prevention.

Plan of Correction: The Program Director is composing a injury prevention procedures.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the applicant did not ensure areas and equipment of the center, inside shall maintained safe. Evidence: During the initial inspection conducted on July 11, 2017, in the unlocked kitchen there were 4 knives accessible. In the School Age classroom, there was a book shelf approximately 6 feet that is a tipping hazard.

Plan of Correction: The center was closed for the kids. No kids were attending. Moving forward the day of opening the kitchen door will remain closed and locked during operating hours.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the applicant did not ensure all hazardous substances were kept in a locked place. Evidence: During the inspection conducted on July 11, 2017 , the licensing inspector observed in the unlocked kitchen and unlocked cabinet hand soap and wall paper paste labeled "Keep out of reach of children" and " Caution ". In the kitchen there was a closet unlocked with hand soap labeled Keep out of reach of children" and " Caution ".

Plan of Correction: The center was closed for the kids. No kids were attending. Moving forward the day of opening the kitchen door will remain closed and locked during operating hours.

Description: Based on review of emergency preparedness plan, the applicant did not ensure all procedural components were in the emergency preparedness plan. Evidence: During the inspection conducted on July 11, 2017, the following components of the emergency preparedness plan was missing in the shelter in place plan secondary means for egress, securing documents and health supplies, facility containment procedures and plan and review and update information.

Plan of Correction: The program director will update document to include shelter in place plan secondary means for egress, securing documents and health supplies, facility containment procedures and plan and review and update information.

Standard #: 22VAC40-185-550-C
Description: Based on observation and review of documentation, the applicant did not ensure emergency evacuation and shelter in place maps were posted. Evidence: During the inspection conducted on July 11, 2017, the shelter in place was not on the maps in the building.

Plan of Correction: The program director will update the maps to include shelter in place.
