22VAC40-191 Background Checks (22VAC40-191)

An unannounced monitoring inspection was conducted at the facility July 12, 2017 from approximately 10:15 pm - 4:00pm. Upon entrance there were 80 children and 9 staff. Additional staff were on site for administrative support. The Licensing Inspector observed bubble activity, returning from field trips, playing video games, watching a movie, and weaving baskets. Licensing inspector reviewed 8 children's records, 9 staff records, 4 medications and 5 injury documents. Exit interview conducted with director.

Standard #: 22VAC40-185-130-A
Description: Based on 1 of 8 records reviewed, the licensee did not ensure each child had a immunization before the child's first day of attendance. Evidence: During the inspection conducted on July 12, 2017, in the record for Child #1 there was no documentation of a child's immunizations.

Plan of Correction: Parents will be contacted tomorrow to bring updated immunization record.

Standard #: 22VAC40-185-140-A
Description: Based on 1 of 8 children's records reviewed, the licensee did not ensure the records contained a physical examination. Evidence: During the inspection conducted on July 12, 2017, the records for child #1 (start date 9/6/2016) did not have a physical examination.

Plan of Correction: Parents will be contacted tomorrow to bring updated physical record.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the licensee did not ensure all hazardous substances were kept in a locked place. Evidence: During the inspection conducted on July 12, 2017, the licensing inspector observed in the boys bathroom in a unlocked cabinet under the sink a hand soap labeled "Keep out of reach of children"and "Caution." In classroom 117, on a shelf there was an aerosol spray and white out on another shelf labeled "Keep out of reach of children" and "Caution." In classroom 133, there was a filing cabinet with polish remover, white out labeled "Keep out of reach of children," "Flammable", "Danger" and "Caution." In classroom 135, there was a bottle of hand sanitizer labeled "Keep out of reach of children"and "Caution."

Plan of Correction: The center removed all hazardous substances from the classrooms and locked the hand soap in a cabinet.

Standard #: 22VAC40-185-550-B
Description: Based on review of the center's emergency plan and interview with the staff the center did not ensure that the emergency preparedness plan contained all required procedural components required by the Standards. Evidence: The center's emergency plan was missing the following: - Establishment of center emergency officer and backup officer to include 24-hour contact telephone number for each - Shelter-in-place inside secondary means of access and egress, securing essential documents and special health supplies to be carried into a designated assembly point. - Drill frequency

Plan of Correction: Policy will be updated to include the information.
