Thank you for your time and assistance with the unannounced monitoring inspection conducted today, July 11, 2107. The on-site inspection was from 10:10 a.m. to 4:00 p.m. On this date, there were 112 children present and in the care of 19 staff members. There were two additional staff members available to assist if needed. There were 156 children enrolled in the center. The exception to compliance was noted on the violation notice and the risk rating for the violation was listed on the supplemental page. The records of ten new staff members and two long-time staff members were reviewed. Ten children's records reviewed and eight classrooms were observed. There were eight staff members and six children interviewed. The medication and medication paperwork were reviewed for five children. Lunch was observed to meet USDA guidelines and consisted of meatball subs, peas, pineapple, and milk. The theme for the week was The Wild West, and the children had been busy making number snakes, cowboy boots and hats. The school-age class was on a field trip to Great Country Farms. Some of items reviewed and observed during the visit included: emergency supplies, first aid kit, posted information, diaper ointments and authorizations, documentation of evacuation drills, injury reports, the playground, choice time, art activities, indoor games, infant feeding and care, diapering procedures, hand washing procedures, bathroom procedures, lunch, and nap time. Description: Based on observation and discussion with a staff member, the center failed to clean a diaper changing pad with soap and water after changing a child's diaper. Evidence: 1. In Classroom #2, when Staff Member A finished changing a child's diaper she did not clean the changing pad with a soap and water solution before sanitizing the pad. 2. Staff Member A confirmed she did not use the soap and water solution before sanitizing the changing pad.

Plan of Correction: Staff Member A cleaned and sanitized the changing pad as soon as the mistake was brought to her attention. The director will retrain all staff on proper diaper changing procedures at the July staff meeting.
