Comments:
The licensing inspectors conducted an unannounced monitoring inspection from 9:30am - 12:00pm. There were fifteen children in care ranging in age from 6 months to 9 years with 5 staff supervising. The children were observed playing on the playground, at story time and during lunch. Five staff and five children's records were reviewed. One violation from the previous inspection had been corrected. Areas of non-compliance are identified on the violation notice. Standard #: 22VAC40-185-140-A
Description: Based on record reviews, the center failed to obtain documentation of a physical. Evidence: 1. The inspector reviewed five children's records. One of five records reviewed (child record #3) did not contain documentation of a physical exam and child's date of enrollment was 01/03/17.

Plan of Correction: The parents will be contacted and a copy of the child's physical will be requested.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the center failed to maintain the outside play area in a safe condition. Evidence: 1. The inspector observed the outside play areas. On the front playground there are several areas where the felt is protruding through the bark mulch creating tripping hazards.

Plan of Correction: The felt will be cut and additional mulch will be placed on the playground in these areas.
