Comments:
The licensing inspector conducted an annual subsidy health and safety inspection from 9:30am - 1:30pm. There were sixteen children in care: three infants with one staff and thirteen children ages 2 to 7 with four staff. One additional staff was also present. The children were observed dancing and listening to music, watching a cartoon and during lunch. Five staff and five children's records were reviewed. Standard #: VENDSUB-000-012-B
Description: Based on record reviews, the vendor failed to obtain documentation of Virginia Preservice Training for staff within 90 days of employment. Evidence: 1. The inspector reviewed five staff records. Two of five records reviewed (staff record #4 and staff record #5) did not contain documentation of preservice training and both staff have been employed for more than 90 days. Staff #4's date of employment was 3/6/17 and staff #5's date of employment was 1/10/17.

Plan of Correction: Staff will complete the preservice training and provide documentation for their records by 09/01/17.

Standard #: VENDSUB-000-012-C
Description: Based on record reviews, the vendor failed to obtain documentation of Virginia Preservice Training for staff who were employed prior to the effective date of the vendor agreement. Evidence: 1. The inspector reviewed five staff records. One of five records reviewed (staff record #1) did not contain documentation of preservice training. The date of the vendor agreement was 1/28/17 and staff employment date was 08/12/14.

Plan of Correction: Staff will complete the preservice training and provide documentation for their record by 09/01/17.
