Comments:
Licensing Inspector arrived at 10:00am and departed at 11:30am to conduct a Subsidy Health and Safety Inspection. At the time of the tour there were 7 children in care with 2 staff present. Children were observed playing ball, during circle time and in infant bouncers. A sample of 3 staff records and 5 children's records were reviewed. The center does not provide transportation.

Violations:
Standard #: VENDSUB-000-031-A
Description: Based on a record review the vendor failed to ensure all componets were included in the emergency preparedness plan. Evidence: The center's evacuation, intruder and shelter-in-place plans did not include reunification plans.

Plan of Correction: The plan has provision to reunify the children to the parents, however it was not titled reunification. The verbiage reunification will be added to the emergency plans.
