Observed 46 children + 10 direct-care staff + 2 administrative staff. Children were observed in a variety of activities, to include: art, play dough, blocks, creative play, outdoor active play, and lunch. Questions about the inspection may be directed to: firstname.lastname@example.org

Evidence: Child #8 - There was no address for the emergency contact. Plan of Correction: There is no field for the online enrollment of back-up care children. We will submit a request for this field to be added to the online form. In the mean time, we will request and write down the address.

Plan of Correction: The solution was moved to a locked cabinet during the inspection.
Standard #: 22VAC40-185-510-N Description: Based on observations made and records reviewed, it was determined that a medication was not disposed of after 14 days of the expiration of the authorization. Evidence: Child #8 - The authorization for an over-the-counter medicine expired on 6/30/17, and the medicine has remained on-site and not been returned to the parent or disposed of. Plan of Correction: The medication will be disposed of promptly.

Evidence: A tube of Desitin in the Toddler I room did not have the child's name on it. Plan of Correction: The product was labeled during the inspection.
