Comments:
An unannounced monitoring inspection was made on 7/24/17 at 9:30 a. m. and concluded at 1:00 p. m. The inspector met with the Program Director and Child Day Center standards were reviewed. The following staff-to-child ratios were observed: Bobcats: (Rising Kindergarten-First Grade) There were 21 children with three staff. The children were engaged in Arts and Crafts under the pavilion. The children were making a jelly fish craft. Wolves: (First-Second Grade) There were seven children with one staff. The children were practicing an "Under the Sea" skit in the Theatre Area. Bears: (Third-Fourth Grade) There were 17 children with two staff. The children and staff were practicing a chant. Eagles: (Fourth-Fifth Grade) There were eight children with two staff and one lifeguard. The children were preparing to put on life jackets prior to getting into the boats. Gators: (Fifth Grade and older) There were were 22 children with three staff. The children were in the Archery area. Seven children records were reviewed. Four medications reviewed. Five staff records were reviewed. Observations were conducted in all camp areas. Conversations were held with several staff and children.

Standard #: 22VAC40-185-60-A
Description: Based on review of children records the center did not have all required information on file. Evidence: On 7/24/17, child #1 record did not include the emergency contact addresses.

Plan of Correction: We will contact the mother to get this information today.

Standard #: 22VAC40-185-510-C
Description: Based on review of medications the center did not follow their policy regarding expired medications. Evidence: On 7/24/17, child #2 had Allergy Relief medication with an expiration date of 4/2017.

Plan of Correction: We will return the expired medication to the parent today.

Standard #: 22VAC40-185-510-D
Description: Based on review of medications the center did not have all authorizations current. Evidence: On 7/24/17, child #2 did not have a current authorization on file from the parent or physician to administer Allergy Relief.

Plan of Correction: We will request that the parent bring in an updated authorization.

Standard #: 22VAC40-185-550-D
Description: Based on review of the current evacutaion log the center did not practice all required drills. Evidence: On 7/24/17, there were no evacuation drills documented for January and February 2017.

Plan of Correction: We will make sure drills are done and documented monthly.
