Standard 550.H was reviewed: The center shall prepare a document containing local emergency contact information, potential shelters, hospitals, evacuation routes, etc., that pertain to each site frequently visited or of routes frequently driven by center staff for center business (such as field trips, pickup/drop off of children to or from schools, etc.). This document must be kept in vehicles that centers use to transport children to and from the center. The new sworn statement or affirmation was reviewed. New CPS check requirements discussed. Anyone who has lived in another state in the past five years is required to request a check of the child abuse and neglect registry for each state they have lived in the past five years. Discussed strategies for labeling infant pacifiers- Refer to Standard 430.I

Comments:
An unannounced monitoring inspection was conducted on 07/19/2017 from 10:15am - 1:20pm. At the time of entrance there were 32 children, ages infant - 11 years, in care with four staff. Children were observed during morning program time, lunch and nap. Records were reviewed for five children and six staff. The inspector additionally reviewed emergency supplies, transportation & field trips, diapering & hand washing, injury documentation and documentation of emergency practice drills. The violation is listed on the violation notice and was reviewed with the program director at the conclusion of the inspection.

Violations:
Standard #: 22VAC40-185-160-A
Description: Based on record review and interview, the center failed to ensure that each staff member shall submit documentation of a negative tuberculosis screening. Documentation of the screening shall be submitted no later than 21 days after employment and shall have been completed within 12 months prior to or 21 days after employment. Evidence: 1. Staff #1 has a hire date of 05/22/2017. Staff #1 does not have a TB screening on file. a. Administrative staff verified that staff #1 had not obtained her TB screening and had worked in the center beyond the 21 day requirement for a TB screening. 2. The TB screening for staff #2 was conducted on 06/12/2017. Staff #2 began employment on 04/10/2017 therefore, the TB screening was conducted after the 21 day requirement.

Plan of Correction: Staff #1 has obtained her TB screening today. Going forward, all new staff will obtain a TB screening within 21 days of employment.
