Standard #: VENDSUB-000-006-B-11 Description: Based on review of children records the center did not have an allergy plan in place for one child. Evidence: On 7/25/17, child #2 had a peanut allergy listed in their record. There was no written allergy plan from the child's physician on file. Plan of Correction: Will contact the parent to get an allergy plan if this is an allergy.
Standard #: VENDSUB-000-012-B Description: Based on discussion with staff the center has not had all staff complete the Preservice training. Evidence: On 7/25/17, some staff have not completed the Virginia Preservice Training. This training is required within 90 days of employment or subsidy vendor appoval. Plan of Correction: We are working on completing this training. Will have it done as soon as possible.
