Comments:
The licensing inspector conducted an unannounced health and safety subsidy inspection Wednesday, July 26, 2017 from approximately 10:25am to 4:00pm. The following ratios were observed with a total of 78 children present during the inspection: Infants - 9 children with 3 staff Toddlers - 6 children with 2 staff Discovery Preschool (Twos) - 16 children with 2 staff Threes- 14 children with 3 staff Pre-K - 10 children with 1 staff School Age - 23 children with 2 staff The children and staff were engaged in a variety of activities during the inspection. The Infants were observed sleeping in their cribs, playing on the floor, having their diapers changed and being fed. The Toddlers were observed having their diapers changed, looking at books, eating lunch and transitioning into naptime. The Discovery Preschool children were observed playing outside and eating lunch. The Preschool children were observed playing outside, playing in centers and eating lunch. The Pre-K children were observed playing outside. The School Age children were observed creating clay pots, playing outside and eating lunch. Lunch consisted of barbecue chicken and rolls, corn, apricots and milk. All areas of the facility including classrooms, hallways, kitchen, playgrounds, and bathrooms were inspected. The facility was equipped with toys and supplies and items were available to the children. Additional subsidy requirements that were not addressed during the monitoring inspection were reviewed. During the inspection, seven children's records and seven staff records were reviewed. Description: Based on review on documentation review and interview, the facility did not have written lock down procedures that including required information. During the inspection, the licensing inspector asked to review the emergency preparedness play. The licensing inspector reviewed the information located in the facility. The facility's lock down plan did not include methods to alert staff and emergency responders and accommodations or special requirements for infants, toddlers, and children with special needs to ensure their safety during the lock down.

Plan of Correction: We will add the required components to the emergency preparedness plan.
