Consultation provided on documenting the time parents are told of injuries/accidents. Make sure to do a shelter-in-place drill this month. Make sure you are updating parental contact information/emergency contacts annually.
An unannounced monitoring inspection was conducted with the director, who was present with 67 schoolagers and 7 staff. Snack was being eaten by one group, which consisted of dried fruit, cheese crackers and water. The Licensing Inspector was able to observe active play, computer time as well as theater time and the older children 13+ watching a movie. Three children had medications on site. Three children and six staff files were reviewed today. Arrival time: 4:20 p.m. Departure time: 7 p.m.
Standard #: 22VAC40-185-80-A Description: Based on a review of the attendance log in the classrooms it was determined that the center is not maintaining attendance to reflect the number of children present in the classrooms. Evidence: There were 11- 5/6 year old children present however 9 children were listed on the attendance. There were 7-9/10 year old children present however 11 children were listed on the attendance. There were 5- 13+ children present however 9 children were listed on the attendance. There were 17- 11/12 year old children present however 20 children were listed on the attendance. Plan of Correction: We will retrain staff today on the need to keep accurate attendance.
Standard #: 22VAC40-185-510-C Description: Based on a review of medications for three children, it was determined that the medication authorization for one child had expired. Evidence: The authorization for the Proair Inhaler for child A had a discontinue use date of June 16, 2017. Plan of Correction: We will obtain a new authorization form.
Standard #: 22VAC40-191-60-C-1 Description: Based on a review of six staff files, it was determined that criminal records background checks have not been obtianed within 30 days of employment. Evidence: There were no criminal records checks on file for staff A, B, C, D, E and F. Staff A, B, D, and E were hired on 5/16/17, staff C and F were hired on 6/19/17. Plan of Correction: I will contact the VA State Police to check on the status of the background checks.
