Comments:
Licensing Inspectors arrived at 10:45 AM and departed at 2:00 PM. At the time of the inspection, there were 75 children (ages infant through school-age) in care with 12 staff. Children were observed participating in outdoor play, doing table activities and napping. Center documents and medication were also observed. The sample size was six staff and seven children's records. Violations were noted in the areas of physical plant and special care provisions & emergencies.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, it was determined that the facility did not ensure that all areas and equipment are maintained in a safe condition. Evidence: There were areas of peeling paint on the door frames of the restrooms and the preschool classroom.

Plan of Correction: All door frames will be re-painted.

Standard #: 22VAC40-185-500-B
Description: Based on observation, it was determined that the facility did not ensure that the diapering surface was sanitized after each use. Evidence: Staff in the toddler room sprayed both soap & water and sanitizer on the changing table at the same time. She then wiped the changing table with a paper towel.

Plan of Correction: Retraining on proper diapering procedures will occur.

Standard #: 22VAC40-185-510-L
Description: Based on review of ointments in the two-year-old room, it was determined that the facility did not have a medication for medications. Evidence: There were two tubes of topical ointments that had completed medication forms. Although staff verified that these medications had been applied, there was no record of the applications.

Plan of Correction: Staff will be retrained to complete medication records.
