Comments:
An unannounced subsidy health and safety inspection was completed on July 27, 2017. The purpose of this visit was to measure the facility?s compliance with some but not all, of the subsidy requirements. There were 46 children, infant to school age, in care at the time of the inspection. There were 7 staff members that were working with the children in their classrooms. All groups were within the proper ratio. The children throughout the facility were engaged in activity. Children in the infant and toddler classrooms were playing with staff on the floor. The 2 year old through four year old classes were returning from outside play, reading books, playing with toys, working in various learning centers and learning with manipulatives. The school age children were engaged in free play. Staff was observed interacting with the children and providing assistance when needed. All areas of the center including classrooms, hallways, bathrooms and playgrounds were inspected. The facility was equipped with manipulatives and supplies and items were available to the children. The following information was reviewed: fire and shelter-in-place drills, liability insurance policy, parent agreements, and staff training information. Medication is administered and multiple staff persons have MAT training.

Violations:
Standard #: VENDSUB-000-005-B
Description: Based on a review of attendance records and sign in sheets, the facility did not ensure that arrivals were documented for each child in care. EVIDENCE: The sign in sheets tracking arrival and departure times for the school age and preschool children were missing arrival times for multiple children in each group who were marked present on the facility's attendance record.

Plan of Correction: We will continue to encourage parents to sign their children in and out. The teachers will check behind the parents to endure that arrival and departure times are recorded.

Standard #: VENDSUB-000-031-B-1
Description: Based on a review of the facility's emergency preparedness plan, the facility did not ensure that the evacuation procedures included all required components. EVIDENCE: The evacuation plan did not include the required procedural components including, but not limited to, procedures to address reuniting children with parents or other designated person.

Plan of Correction: We will update the missing requirements today to make sure that the plan is complete.

Standard #: VENDSUB-000-031-B-2
Description: Based on a review of the facility's emergency preparedness plan, the facility did not ensure that the shelter in place procedures included all required components. EVIDENCE: The shelter in place plan did not include the required procedural components including, but not limited to, procedures to address reuniting children with parents or other designated person.

Plan of Correction: We will update the missing requirements today to make sure that the plan is complete.

Standard #: VENDSUB-000-031-B-3
Description: Based on a review of the facility's emergency preparedness plan, the facility did not ensure that the lockdown procedures included all required components. EVIDENCE: The lockdown procedures did not include the required procedural components including: methods to alert emergency responders, methods to account for all children in a lockdown location, methods of communication with parents and emergency responders, accommodations or special requirements for infants, toddlers, or children with special needs and procedures to address reuniting children with parents or an authorized person designated by the parent to pick up the child.

Plan of Correction: We will update the missing requirements today to make sure that the plan is complete.
