Technical Assistance:
Technical assistance was provided in the following areas of the standards: 22VAC40-185-(6)-390 (School age activities); 22VAC40-185-(7)-550 (Emergency Drills); 22VAC40-185-(8)-580 (Transportation and field trips)

Comments:
An unannounced monitoring inspection was conducted on 7/31/17 from 10:25am - 2:30pm. During the inspection there were 115 children ages seven months old to twelve years old in care with 15 staff. A tour of the facility was conducted and children were observed playing outside, participating in various classroom activities, eating lunch, and during rest period. Records were reviewed for ten children and eleven staff. There is no medication at the facility. Emergency procedures, and emergency supplies were reviewed. Areas of non-compliance are identified on the violation notice, and were discussed during the exit interview.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, it was determined that all areas and equipment of the center, inside and outside, shall be maintained in a safe and operable condition. Evidence: 1. There is brown plastic house on the Two year old playground that has cracked areas around the window where a child could get injured. 2. There was hose lying on the ground outside the Pre-K classroom that presented a tripping hazard. 3. Staff #11 (Assistant Director) confirmed that all areas and equipment were not in a safe and operable condition.

Plan of Correction: The facility responded: The play house was removed the playground and the hose was placed in a container for storage during the inspection.

Standard #: 22VAC40-185-320-B
Description: Based on observation, it was determined that the provider did not ensure that the running water from the sinks does not exceed 120 degrees Fahrenheit. Evidence: The water temperature at the sink in the Preschool room measured 128.1 degrees Fahrenheit.

Plan of Correction: The facility responded: The water temperature was turned down during the inspection.

Standard #: 22VAC40-185-330-F
Description: Based on observation, it was determined the the facility did not ensure that a shady area shall be provided on playgrounds during the months of June, July, and August. Evidence: 1. There was no shady area for the children using the Toddler playground. 2. Staff #11 (Assisatnt Director) confirmed that currently there was not a shady area for the Toddler playground.

Plan of Correction: The facility responded: The previous shade structure was damaged and the new shade structure will be installed this week.

Standard #: 22VAC40-185-500-B
Description: Based on observation, it was determined that the facility did not ensure that a nonabsorbent surface for diapering or changing is used. Evidence: 1. The changing pad on the diaper changing table in the Toddler classroom had a one inch tear on it. 2. Staff #11 (Assistant Director) confirmed that there was a tear in the diaper changing pad.

Plan of Correction: The facility responded: A new diaper changing pad was placed in the Toddler classroom during the inspection.
