Discussed the recent communication regarding fingerprint based background checks.
The licensing inspector conducted an unannounced monitoring inspection Tuesday, July 25, 2017 from 09:45am to 12:30pm. The children were observed playing outside on the playground and in the bounce house/slide. The infants were observed being fed and sleeping. The schoolagers were observed playing with various games. Lunch consisted of spaghetti, broccoli, carrots, cantalope and milk. A menu was posted. All classrooms and playgrounds were inspected today as well as the facility?s mini buses. Eight children?s records and eight employee records were reviewed during this inspection. The center administers medication; however they did not have any being given at this time. Total enrollment today: 84 Today, the following child to staff ratios were observed: Infant I ?7:2 Infant II- 4:2 Toddler A & B ?16:3 Younger Twos ? 13:2 Older Twos ? 8:2 Threes ?15:2 School Age ?29:2 The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-140-A Description: Based on a review of children's records on 07/25/2017, the center did not ensure that one of feight records reviewed contained documentation of a physical examination before the child's first day of attendance or within one month after attendance. EVIDENCE: The record of Child #4, who started at the center on 01/04/2016, did not contain documentation of a physical examination. Plan of Correction: Will email mom to get physical by Friday.
Standard #: 22VAC40-185-60-A Description: Based on a review of children's records on 07/25/2017, the center did not ensure that one of eight records reviewed contained the work phone number and place of employment of each parent. EVIDENCE: The record of Child #2 did not have a phone number for the place of employment listed for the Child's father. Plan of Correction: Will email parent today to get missing number.
Standard #: 22VAC40-185-280-B Description: Based on observation on 02/25/2017, the center did not ensure that all hazardous substances such as cleaning products were kept in a locked place. EVIDENCE: 1. An unlocked cabinet with several bottles of cleaning supplies was observed in the Toddler room. 2. Several bottles of cleaning supplies were observed in an unlocked cabinet in the Younger Two's bathroom. 3. An unlocked cabinet with two bottles of cleaning supplies was observed in the Older Two's room. Plan of Correction: Will get more locks for cabinets today.
Standard #: 22VAC40-185-570-E Description: Based on observation on 07/25/2017, the center did not ensure that prepared infant formula was dated. EVIDENCE: A bottle of prepared formula belonging to Child #6 was observed in the refrigerator in the Infant I room. The bottle did not have a date on it. Plan of Correction: Will remind teachers to check the bottles. Will email infant families a reminder today.
