Technical Assistance:
Technical assistance today included medication authorizations and allergy action plans; and including a visiting child in the ratio and documented on the attendance.

Comments:
An unannounced monitoring inspection was conducted today from 1:00 pm - 2:45 pm. There were 30 children present with 6 staff. I observed children eating lunch; preparing for nap and reading books. Violations:
Standard #: 22VAC40-185-70-A
Description: Based on a review of records today, some required staff information was not available. Evidence: There was no documentation of orientation for Staff # 3; and no documentation of tuberculosis screening for Staff # 2 (hired 1/17) and Staff # 3 (hired 5/30/17).

Plan of Correction: We will get the documentation in the staff files.

Standard #: 22VAC40-185-270-A
Description: During today's inspection, some areas and equipment were observed to not be maintained in a safe condition. Evidence: In the playground today, the dome structure was observed to be not attached to the ground. An area of the lattice was broken and a hole in the ground in one area had an exposed root, creating a tripping hazard.

Plan of Correction: The dome was not attached because we just put fresh mulch on the playground. We will put stakes/outdoor staples so it is stable before any children use the dome. We will put duct tape on the lattice today. The hole is from where children have been digging, so we will block it today and eventually fill it will mulch.

Standard #: 22VAC40-185-330-B
Description: During today's inspection, some outdoor playground equipment was observed to not have resilient surfacing in fall zones. Evidence: The "airplane" climbing structure was observed to be on grass with no resilient surface in fall zones.

Plan of Correction: We will get some mulch in that area.

Standard #: 22VAC40-191-40-D-1-B
Description: Based on a review of staff files today, there were some required background checks were found to be incomplete. Evidence: Staff # 2 had the Search of the Central Registry without results documented. The Criminal Background check was from when the staff was at a previous child care center, and these are not transferable. Staff # 3 did not have a completed Sworn Disclosure Statement within 30 days of employment; and the Search of the Registry did not have results documented within 30 days.

Plan of Correction: The Registry Search forms were returned and sent back within 30 days but we have not received them. We are having trouble with mail reaching us and will look for other ways to receive our mail. These will be completed as soon as possible.
