Comments:
An unannounced monitoring inspection was conducted today from 2:20pm-3:50pm. There were 109 children directly supervised by 15 staff. The physical plant, 5 staff records, 5 children?s records, 6 medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed returning from field trips, washing hands and transitioning to snack. There was an abundant supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Violations:
Standard #: 22VAC40-185-140-A
Description: Based on record review, one child did not have documentation of a physical examination by or under the direction of a physician. Evidence: Child #3's record did not contain documentation of a physical examination by or under the direction of a physician.

Plan of Correction: We will request this information from the child's parents.

Standard #: 22VAC40-185-280-B
Description: Based on observation, not all hazardous materials were kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. Two canisters of sanitizing wipes, and two spray bottles of cleaner were observed in an unlocked cabinet under the sink in the Performers 1 classroom. 2. A canister of sanitizing wipes was observed in an unlocked cabinet in the Art 1 classroom.
