The licensing inspector conducted an unannounced complaint inspection on 7/20/2017, in response to a complaint that was received by the licensing office on 7/12/17. The inspector was on site from approximately 12:37PM to 2:00PM. Interviews were conducted with staff and observations were made relating to allegations concerning ratios, physical site, rest and equipment. The information gathered and observations made during the investigation supports the allegations, so the complaint is determined to be valid. Please complete the columns for ?description of action to be taken? and ?date to be corrected? for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office within 5 calendar days of receipt. Complaint related: Yes
Description: Based on observations at the facility, the facility did not ensure that children's belongings were each in their individual place. EVIDENCE: The blankets for the school age children were located together on a table.

Plan of Correction: Children will use hooks to hang and store their belongings.

Standard #: 22VAC40-185-440-A
Complaint related: Yes
Description: Based on observations at the facility, the facility did not ensure that all children were provided with cots, rest mats or beds during rest time. EVIDENCE: The inspector observed 3 of the 18 children in the group laying on the floor without a cot or mat during a period of rest. Two of the children were laying on blankets and the third was laying directly on the floor.

Plan of Correction: Corrected on site. I will train staff again on the proper rest time procedures.

Complaint related: No
Description: Based on observations at the facility, the facility did not ensure that a cot used by a child had linens consisting of a top cover and a bottom cover. EVIDENCE: The inspector observed a child in the preschool group laying on a cot, at rest time, without a top or bottom cover.

Plan of Correction: Corrected on site. I will speak with the staff to make sure that each child has a blanket and a sheet.
