Comments:
An unannounced renewal inspection was conducted today August 1, 2017 beginning at 9:20 A.M. and ending at 12:00 PM. There were thirty-seven children present today with five staff. The inspector reviewed five children's and five staff's records during the inspection. The children were observed during: free play, outside, lunch, and game time. Discussion with site director about new sworn disclosure statement, using a injury report form for each injury instead of a log for all injuries.

Violations:
Standard #: 22VAC40-191-40-D-1-C
Description: Based on review of five staff's records, the center failed to ensure that all backgrounds were completed within the 3 year time frame. Evidence: 1. Staff # 5 most recent CRC was 5/5/2014. The CRC is requirerd to be updated every three years.

Plan of Correction: The site director sent the staff person today to complete a new CRC form and it will be sent off tomorrow.
