An unannounced inspection was conducted at the facility on August 4, 2017, from approximately 10:00 am - 1:45 pm. Upon entrance there were 31children and 6 staff . The Licensing Inspector observed children outside, and playing with manuipulatives. The Licensing Inspector reviewed 5 children records and 5 staff records. Medication is not given at the center. The exit interview conducted with owner/director.
Standard #: 22VAC40-185-130-A Description: Based on record review, the licensee did not ensure the center obtained immunization documentation for each child before the child attended. Evidence: During the inspection conducted on August 4, 2017, the records for child #1 did not have a immunization record. Plan of Correction: The licensee will follow up with the legal guardian.
Standard #: 22VAC40-185-140-A Description: Based on 1 of 5 children's records reviewed, the licensee did not ensure the records contained a physical examination. Evidence: During the inspection conducted on August 4, 2017, the records for child #1 did not have a physical examination. Plan of Correction: The licensee will follow up with the legal guardian.
Standard #: 22VAC40-185-60-A Description: Based on 2 of 5 records reviewed, the licensee did not ensure the each child's record contained documentation of required information. Evidence: During the complaint investigation conducted on August 4, 2017, the following records did not contain the following information: 1- Record for child #1 did not have a complete address for both emergency contact phone number and did not have proof of identity verification. 2- Child #4 did not have proof of identity verification. Plan of Correction: The licensee will speak to each person this evening.
Standard #: 22VAC40-185-550-B Description: Based on observation, the licensee did not ensure the emergency preparedness plan contained all procedural components. Evidence: During the inspection conducted on August , 2017, the center's emergency plan was missing the following: -Sounding of alarms - Evacuation plan assembly points and primary and secondary means of egress. - Staff training requirement, drill frequency and plan review and update. Plan of Correction: Review over requirements for emergency preparedness plan and update emergency preparedness plan.
Standard #: 22VAC40-185-560-G Description: Based on observation, the licensee did not ensure the food container shall be sealed and clearly dated and labeled in a way that identifies the owner. Evidence: During the inspection conducted on August 4, 2017, the Licensing Inspectors observed 4 of 4 lunch boxes were not clearly dated and 3 of the 4 lunch boxes were not labeled in a way that identifies the owner. Plan of Correction: All names will be written with permanent marker on lunch boxes and date tags will be issued.
